This tool was originally designed by the Lutherans Concerned/North America and then adapted by the Reconciling Methodists and was revised for a Jewish context by Hebrew Union College. It has been edited by SOJOURN.

Remember: this is a survey for several different denominations in Judaism. Different questions will be experienced differently by members of each denomination. Please be sure to read the entire survey before starting so answers are not repeated.
